A Step-by-step approach for Organizations New to the

| Step 1. |
Review the Q and A (Questions and Answers)
page and the
Philosophy, Principles and Beliefs page to determine whether the benefits to your
industrial site will justify the costs in both time involved and money. |

Step 2. |
Get permission from the most senior front line manager accountable for the
safe and productive operation of powered industrial trucks. Permission starts with the signing of the National LIFTOR Industrial Site Licensing
Agreement and Purchase Agreement. [This is the person accountable for
operator injuries as well as collateral losses. They usually have enough
policy-level authority to authorize a purchase order for the required
Industrial Site Licensing Agreement with National LIFTOR. Typical job
titles are Vice President of Materials Control, V-P Distribution, Plant
Manager, Director of Operations, V-P of Administration, Warehouse
Manager, Distribution Manager, etc. |

Step 3. |
Click here to register your intent with National LIFTOR Headquarters
and to request an "Installation Packet". There is a $200 nonrefundable deposit for the Installation Packet.

Certification Packet with selected Operator Learning Objectives,
Certification Test Items, a supply of "Daily/Shift Examination Record" for
inspecting lift trucks, and a toll free 24/7 hotline number you can call
for helpful discussion or to get any of your questions answered about
how to use National LIFTOR at your industrial site.] |

Step 4. |
Select an industrial site Team with a manager who will act as the National LIFTOR
Leader at your industrial site. The initial LIFTOR Team usually
includes:
- 3 operators (already experienced with lift truck operation. must be
able to read at 8th grade level and participate in initial validation
and reliability meetings)
- 2 Front-line supervisors, and
- 1 Senior Manager who "champions" the Team's purpose.
- 1 Registrar from National LIFTOR headquarters will be provided for
facilitation, consultation and oversight.
The senior manager or champion normally has front
line accountability for industrial truck operators, their day-to-day
productivity and their supervision. They usually are accountable for
materials warehousing, logistics, and/or shipping or distribution.] Their role is
to act as a "Team Leader", help, through their team, to
validate training and testing materials from National LIFTOR, schedule the early stages of the
National LIFTOR installation, assign additional front-line supervisory personnel as
instructor/test examiners, provide ongoing clear expectations to all
participants, as well as to provide equipment and other resources. Also
provides weekly/monthly communications on how well or poorly safe
practices are emerging at your industrial site, and uses the vast
informational resources at National LIFTOR headquarters via phone, fax
and email. |

Step 5. |
Schedule a "walk-through" consultation with a National LIFTOR
registrar who will recommend improvements or changes to equipment,
maintenance, engineering, and/ or supervisory practices. A baseline
account of industrial truck related injuries and incidents will be recorded, as well
as any significant collateral damage within the last three years.

This may take 2-3 days to initially align the LIFTOR Team and the
National LIFTOR Registrar. This time is also used for planning,
customizing and validating the training and testing documentation
provided by National LIFTOR. |

Step 6. |
Schedule the National LIFTOR Master Test Examiner to provide OPERATOR
certification to the candidates for Certified Instructor&Test Examiner [Depending on skill levels, this
typically takes as little as one day or as much
as one week. Please avoid delays or extra costs to you by making sure that ALL equipment
is in safe condition with NO defects. No Instruction or Testing will be conducted
using defective equipment.] |

Step 7. |
Schedule the Certified Instructor/Test Examiner Workshop and order
Instructor/Test Examiner Kits [Workshop is 4 days on-site, or at a
location of your choice near your industrial site]. |

Step 8. |
Within 4 weeks of Workshop completion, schedule on-site Certification
Test of Instructor & Test Examiner Candidates ("Acid Test"). Each
Candidate has to
demonstrate that they can administer the on-truck OPERATOR
performance test to the criteria required for National LIFTOR
Certification status. This has to meet the approval of the Master Test
Examiner who observes the candidate I/TE testing his/her operator [For
each Instructor &Test Examiner Candidate, the "acid test"
typically takes as little as about 2 hours and as much as one full week.] |

Step 9. |
Certified Instructor/Test Examiners request a customized certification
packet, from National LIFTOR headquarters or through
www.LIFTOR.com, for each operator they
attempt to certify. After each operator is successfully tested, documents are
forwarded for credentials (wallet-size ID card and Certificate issued
by National LIFTOR). |

Step 10. |
Progress reports are sent quarterly or on demand (Soon available via web
download) Telephone, monthly
teleconference, email and
fax communications are ongoing to make sure that the risks associated
with industrial truck operation are continuously improving. All
consultation over phone, teleconference, fax and email is included in industrial site
license. The only additional fees are for any on-site
consultation, training, skill certification support, or additional materials you request. |
